Annual maximum of $1,500 or $2,000
100/90/60 In Network and 100/80/50 Out of Network
Ortho for groups of 10+ available
Endo/Perio can be quoted in either Basic or Major
Utilizes the First Dental Health PPO network – Over 12,000 dentist locations -- www.firstdentalhealth.com
Groups without current dental coverage may be quoted with waiting periods waived
Plans are based on a Benefit Year
